2. No alcohol/controlled substance use, - or any substance listed on the ETG Agreement -- With the exception of prescribed medications., 06/09/2025
3. Random testing, - You must be available for home visits and random testing by Probation and/or law enforcement. You must also allow a brief search of your home to confirm that you do not have alcohol and prohibited substances in your home. A curfew may be imposed.
- If there is a Positive test, a Refusal to submit to testing, or Tampering of any kind with the sample or the test, then Probation may require you to enter Treatment and/or Complete a Comprehensive Assessment with Probation as collateral and follow all Recommendations, 06/09/2025
4. Provide List of Prescribed Medications to Probation Officer, 06/09/2025
5. Sign all releases of information, 06/09/2025
6. Obtain Permission from Agent before leaving the State, 06/09/2025
7. Give a DNA sample when directed., 06/09/2025

- Drug Court - Call your Probation Officer within 48 hours of sentencing and /or release from custody at [REDACTED_PHONE]

9. Follow all Conditions set forth in the Probation Agreement, 06/09/2025
10. Enter and successfully complete Drug Court, - Obey all Rules of Drug Court.- You will remain in custody until programmig has been arranged by the Treatment Court Team.
- Appear for Judicial Reviews as directed by the court or probation. You must be on time.
- You are only allowed to use one Hospital, one prescribing Primary Physician, one Psychiatrist or Mental Health professional (if needed) and one Pharmacy
- If you are absent from the Drug Court program for more than 60 days you will be Suspended and must petition to be reactivated.
